Such differences may be explained, in part, by the greater comorbidity observed among female patients as well as the potential under-use of appropriate medications and procedures among older and female patients. We investigated the prognostic significance of patient sex and other characteristics for mortality and re-hospitalization 1-year post-discharge among a large well-defined cohort of patients admitted to hospital with AMI in Calgary, Alberta during 1998 99. METHODS: Detailed clinical data for all patients admitted to 3 teaching hospitals in Calgary, AB with a recorded primary discharge diagnosis of AMI in the fiscal year 1998 99 n 914 ; were collected via a standardized and comprehensive medical chart review. Data elements included indicators of patients' sociodemographic and clinical characteristics, the administration of cardiac medications and procedures, consultations to allied health services and in-hospital mortality. Linkages with hospital administrative records and Vital Statistics were performed to capture details regarding re-hospitalization events and mortality during 1-year post-discharge the index AMI event. Multivariate logistic regression models were employed to examine predictors of 1-year mortality and re-hospitalization following adjustment for relevant confounding and or effect-modifying factors. RESULTS: Compared with males, female patients were significantly more likely to be re-admitted to hospital 67% vs. 51% ; , to experience multiple re-hospitalizations 20% vs. 14% ; and to die 17% vs. 12% ; during the first year after the index AMI event. Adjusting for age, comorbid conditions and cardiovascular procedures, the odds ratio for re-hospitalization for women was 1.78 1.29-2.45 95% CI ; . For the endpoint of 1-year mortality, female sex did not remain a significant independent predictor after adjustment for age, social factors, comorbid conditions and cardiovascular procedures. Other significant predictors of 1-year mortality included age 75 years, living alone, congestive heart failure, peripheral vascular disease and chronic renal disease. CONCLUSIONS: Following adjustment for a comprehensive list of detailed clinical and social factors, female sex was no longer observed to represent a significant risk factor for 1-year mortality following AMI.
